Adult Day Programs
Adult Day Programs provide stimulating activities for eligible individuals 18 years or older referred by Ontario Health atHome, and offer caregivers support and respite.
Eligibility
York Region’s Adult Day Programs are for individuals who are 18 years or older and:
- Are referred by Ontario Health atHome
- Have a valid Ontario health card
- Live in a community setting
- Can adhere to our procedures for medication reminders
- Would benefit from socially interacting with others
Individuals are not eligible if they require the following:
- Two-person transfers
- Extensive assistance with personal care (e.g., changing incontinence products, personal cleansing during toileting, consistent one-on-one support requirements, assistance with eating etc.)
- Management of severe disinhibited behaviours or disruptive agitation
Management of any behaviours that are threatening to other clients or staff, or place the client or any other person at risk
Please note: The use of restraints and isolation are not allowed within the Adult Day Programs

The Meeting Place | Monday to Friday 7:30 a.m. to 6 p.m. | For adults over the age of 18 with (including Alzheimer’s disease) and other cognitive impairments.

Aphasia Program | Wednesday 9 a.m. to 3:30 p.m. | For adults 18 years and older living with aphasia or related communication disorders.

ABI Program | Tuesday and Friday 9 a.m. to 3:30 p.m. | For adults 18 years and older, living with the effects of an acquired brain injury.

The Club | Monday and Thursday 7:30 a.m. to 6 p.m. | For adults over the age of 18 who live with enhanced cognitive impairments and physical disabilities, socially isolated, or diagnosed with another age-related illnesses and could benefit from some support and supervision.

Referrals
To be referred to our Adult Day Programs, contact Ontario Health atHome Support Services at 1-888-470-2222.
Referrals can be made for adults over the age of 18 who are frail, socially isolated, or diagnosed with Alzheimer’s, dementia, an acquired brain injury or Aphasia, or other age-related illnesses and could benefit from some support and supervision.
Cost
Each program has a daily fee for service. Subsidies are available for qualified participants upon intake to the program. Please contact Christina Burns at @email or 1-877-464-9675 ext. 76079.
Additional details
- Families must advise program staff of any medications to be taken during program hours. Staff will safely store medications and remind clients to take their medications by themselves as part of our safe medication procedures
- A nutritious lunch and snacks are served daily
- Sample daily activities include card games, music sing-alongs, exercises, arts and crafts, musical performances, theme days and more
- Families are encouraged to drop off and pick up their loved ones or arrange transportation with York Region Transit Mobility On-Request. Our staff can provide you with the Mobility On-Request application or you can download it at yrt.ca
- Participants on a waitlist to join the Adult Day Programs will be offered the opportunity to join virtual programs to socialize and engage with other clients
